Notification does not exist

We have some orders with multiple notifications in the object list and some of these notifications do not actually exist in QMEL (error IM468). The order canot be progressed because whenever we try to save it, the error appears.

We have found SAP note 377265, which references SAP note 379967 to delete these entries from the object list.

However we would like to know what could possibly have caused this to happen.

This is the symptom described in note 377265:

When you create notifications from the object list and you exit the notification with Cancel or Exit, it can happen that an entry for the notification remains in the object list. For a subsequent change transaction, you may experience a dump or update terminations.

I tried doing that - create a notification from the object list - but the exit and cancel buttons are grayed-out, only the back button is active. So I dont think this is what had cause the problem.

Any ideas? Is this related to number range buffers? Or perhaps the early number allocation in the configuration of notification type?

We are running on ECC6.

We had something similar on my previous project.

It turned out that it was one of our user-exits in the order causing the problem at save..
