Task Follow-up Action in Notification

Hello All,
In my notification, there are items tab/associated task tab(IMG names: 20\TAB01 & 20\TAB03). I also have a predefined task YYYY which have a follow up action XXXX assigned.

I met an issue when the below scenario takes place:
There were 2 items and both of them had the YYYY task existing at the some point, i completed the first one YYYY task and it successfully excute the follow up action XXXX and task status changed to TSCO FACU. But... at the moment, you would see that the other YYYY task has 'FACU' indicator in status field though i did not complete this task. Also, when i tried to complete the second YYYY task, i failed to execute the follow-up action.


Does this mean the follow-up action can be used only one time in a notification for the same task?
