Forwarded Workflows Disappearing

Hi,
We have a strange situation with our workflows when they're forwarded. If the original person decides they want someone else to process the workflow they use the Forward button in their SAP Business Workplace and enter the other person's user id. They click on the tick and the work item is removed from their inbox. The problem is sometimes it doesn't end up in the other person's inbox either.

I can usually find them and do an administrator forward from within SWI2_ADM1, however this seems to be an ongoing issue.

We have not done any modifications to SAP Business Workplace and I can't find anything on the net anywhere on how to resolve this.

Has anyone else come across this and how was it fixed ?
