The problem is the functionals have over-granted permissions, many users have access to sm51 and db13 meaning they can shutdown the system so the functionals have done a sloppy job. Turns out the code page converter cache was too small. This is controlled by rsts/ccc/cachesize as per documentation in notes 1946669 and 99699.

Nope, I copied over the /sapmnt/PRD and /usr/sap filesystems from our production app server, which is not Unicode. The locales match.

What OS?
Have a look at your Oracle log ,this is named alert_<SID>.ora and is under your $ORACLE_HOME ,if on Unix, or %ORACLE_HOME% if on Win. After the database startup messages you should have no significant ORA- codes, if any, post them.
Also your paging space could be low.
